We are seeking a Full Time Nurse Case Manager to enhance the continuity of patient care by serving as a liaison between patients, hospital partners, referral sources, Patient Services Coordinators (PSC) and Medical Scribes. The Case Manager stays current with all company changes and effectively communicates with company and facility staff regarding center and business operations. In this role you will be responsible for reporting wound outcome case reviews; consisting of weeks in treatment, number of visits, summary of wound measurements, current care plan, care plan changes and/or challenges. The ideal Case Manager demonstrates strong analytical and problem-solving skills, and promptly reports any issues to the designated supervisor. What We Offer:Monday – Friday schedule, no weekendsComprehensive benefits package (Medical, Dental, Vision, Life, 401k)Generous paid time offInternal ongoing educational opportunitiesWhat You’ll Do:Provides information by responding to queries from patients, hospital partners, referral sources, PSC, and Medical Scribes by sorting and distributing message and documentsDocuments all communication in EMR system according to company policyProvides patient education relevant to disease processes, medications, pressure reduction, and nutrition/vitamin therapyCompletes accurate clinical notes regarding patients’ condition and care given. Writes progress report and summaries to Providers as neededChecks on patients in Acute Care, Skilled, Hospitalized and LTAC facilities several times a week and updates accordinglyCoordinates patient care; orders Home Health, IV ABX Therapy, consults to a specialist, etc.Qualifications:Valid Registered Nursing license requiredMinimum 3 years of nursing experiencePrior experience working with wound care patients highly preferredIntermediate Microsoft Office skills (Word, Excel, Outlook), technologically savvyEffective problem-solving skills, decision-making via innovation & creativityAbility to maintain confidentiality, per HIPPA regulationsAt RestorixHealth, we grow talent.
