Position: RN/Critical Care Hours: Variable The essential duties and responsibilities of a Critical Care RN for the CISCU HV Department includes: -Supporting patient safety initiatives by maintaining an environment of safety for patients, self and others -Complying with HIPPA regarding patient protected health information (PHI) -Ensuring that reports are given using the SBAR format and utilizing SBAR Ticket to Ride when transferring patients -Remaining courteous and professional in all interactions with patients, visitors, and staff from other departments -Making an extra effort, showing good follow through in meeting the patient's/family's needs, responding promptly to their needs regardless of the assignment and delivering patient care without making an excessive amount of noise -Coordinating the placement of patients with Patient Reservations/the Administrative Coordinator throughout the shift -Evaluating the patient assignments at the beginning and throughout the shift for appropriateness, and basing oncoming shift assignments on patient acuity needs and staff qualifications -Evaluating and adjusting the staffing throughout the shift to accommodate any increase or decrease in patient acuity or census, communicating with the Nursing Supervisor and the Manager regarding staffing adjustments, reviewing the staffing pattern for the next shift to ensure adequate coverage, and following the scheduling policy guidelines for staffing levels, mandatory call-ins, low census, on call shifts and floating to other units -Communicating appropriately and in a timely manner with coworkers concerning changes in assignments and/or admissions/transfers -Directing activities to ensure unit coverage during the staff's meals/breaks, sedation for patient procedures, and transportation of patients to special procedures such as CT or MRI -Assisting with admissions, discharges and transfers as needed -Verifying that all vacant patient rooms are prepared and equipped for admission -Upon admission and per protocols, completing assessments of the patients condition, including their biophysical, psychosocial and age specific needs -Providing a clear and organized review of this assessment data Documenting the patient's status according to the unit specific, and nursing division policies and procedures -Collaborating with interdisciplinary team members to develop and implement an individualized plan of care (POC), while addressing utilizing the nursing and medical diagnosis -Evaluating the patient's status and appropriateness of the plan of care -Ensuring that the POC and/or pathway is consistently initiated upon admission, that problems are prioritized and that the problems are resolved appropriately on a daily basis Identifying and initiating independent nursing interventions to deal with any potential complications, and communicating with the physicians when problems do occur -Ensuring that orders are verified correctly and completely, and that they are signed off prior to the end of the shift via cosigned chart check and "Nurse Review" of all electronic orders -Resolving any narcotic discrepancies, clarifying medication orders prior to the end of the shift, administering the medication using two patient identifiers such as the patient's name and their medical record number, verifying High Alert Medications by a second nurse prior to administration, and explaining the medication to the patient, such as the name of the medication, its action and its side effects -Providing patient and family education according to the patient's individualized needs, and as delegated by the physician and/or hospital procedure, and documenting any education provided to the patient/family into the RCARE Education Record at least once per shift -Completing charge nurse checklists, and ensuring that PCT tasks and all care is completed by the end of the shift -Completing the daily staffing report and verifying that the patient census and accommodation codes are correct -Supporting a teamwork and a positive work environment on the unit by engaging in open and positive communication with peers, and displaying a willingness to assist others -Demonstrating autonomy in finding solutions to problems, and effectively and appropriately delegating tasks to coworkers -Assisting the staff in finding the answers to their questions, and serving as a clinical resource to CTRU nurses, other critical care units and the 4W staff when needed -Maintaining attendance standards by consistently clocking in and out no earlier than 7 minutes before the beginning of the shift, being ready to receive reports at 6:45, following assigned work hours, limiting absences and tardiness, and providing proper absence notifications -Attending at least 50% of the annual staff conferences during the fiscal year -Meeting the requirements for annual compliance training, licenses, certification (ACLS/BLS) renewals, and annual TB testing without reminders or follow-ups -Participating in continuing education activities, conferences and/or classes to improve nursing practice -Supporting the education process for support personnel and new hires and being willing to function as a preceptor Supporting performance and quality improvement by participating in unit based and/or hospital wide committees or councils -Recognizing, supporting, and utilizing evidence based nursing practices and adhering to the American Nurses Association Code of Ethics -Demonstrating knowledge of policies, procedures, guidelines and protocols Following hospital and unit specific policies and procedures Other duties may be assigned.
