Education Optometrist Statement of Work The optometrist (contractor) must have a Doctor of Optometry (OD) degree and be a licensed optometrist in any state. The provider shall furnish a copy of all state licenses, certifications, and/or registrations verifying the appropriateness of the provider to render such services.Contractor will be required to attend quarterly contractor training sessions and other training sessions as needed. The contractor shall be paid for such sessions.Scope of ServiceThe contractor shall be responsible for:The provider shall deliver services to include, but not limited to, the examination, diagnosis, and treatment of eye disorders, recommendations to the Clinical Director for appropriate optometry care, treatment for ocular disease, prescribe eye wear and any medically indicated features of eye wear for patients' unique ocular needs, perform tonometry and slit lamp examinations when medically indicated/requested, and make necessary adjustments of institution eyeglasses and frames. The provider shall make recommendations to the Health Services Administrator and Clinical Director for the treatment of eye diseases and the procurement of eyewear. The provider will follow standard infection control practices, including the mandates of the concept of Universal Precautions and applicable OSHA guidelines. The provider is required to have extensive knowledge of ocular conditions and diseases. All care provided to the patient shall be commensurate with community standards.The provider shall deliver written reports of clinical findings and recommendations for treatment. The provider shall provide continuing medical education to the Health Services staff as required and scheduled by the Health Services Administrator.The provider will assist in monitoring the attendance of inmates on call-out to the Health Services Department for care and treatment. Reports will be made to the Health Services staff of any inmate infraction of institution rules and regulations with respect to their interaction with the provider.
