Location: LBHU Employment Status: FT----SUMMARY:--Uses skilled intervention, clinical best practiceknowledge and collaboration with the patient, physician and multidisciplinarytreatment team to concurrently pace care, achieve quality outcomes andnegotiate appropriate reimbursement. They ensure thetimely and seamless clinical transition of patients through medicallyappropriate levels of care.ESSENTIAL DUTIES AND RESPONSIBILITIES:1.------------ Represent LGH to our customers (patients, familymembers, significant others, physicians, etc.) as a concerned, compassionate,and efficient healthcare organization.2.------------ Support LGH's goals and objectives.-- Represent LGH to external parties in aneffort to enhance the image of the organization.3.------------ Create a working relationship with the medical staffmembers, employees, and management of LGH in an effort to accomplish thedesired goals and objectives of LGH and the Case Management department.4.------------ Ensure proper and complete documentation ofassessments, interactions, interventions, and outcomes.5.------------ Provide intensive counseling and crisis interventionas needed.6.------------ Report or assist with reports of suspected child/adult/elderlyabuse/neglect.7.------------ Assist and refer eligible patients in applying forand/or acquiring financial assistance.8.------------ Complete psychosocial assessment including mentalstatus and DSM IV diagnosis on Axis IV and V as appropriate to area ofpractice. 9.------------ Ensure appropriate post-acute placement ofpatients.-- Discharge planning will bestarted at the time of admit, and will be an ongoing multi-disciplinarycollaborative effort. 10.------ Demonstrate the knowledge and skills necessary toprovide care or support as appropriate to the age of the patients serviced inhis or her assigned area(s).11.------ Make timely and appropriate referrals to outsideagencies (i.e. hospice) as identified.12.------ Act as hospital representative for all adoptions whileensuring all legal procedures are followed, and confidentiality of all partiesinvolved is ensured.13.------ Assist in orienting new employees/interns asappropriate.-- May be asked to act as atask and/or LCSW supervisor in charge of students participating in social workinternships.14.------Exposure and access to confidential information on adaily basis.-- Information includesmedical records, insurance information, billing information, and physicianprocess. 15.------Continuous responsibility for items of extremevalue.-- Requires close attention to avoidsignificant loss.-- Responsible for thecare and security of computers which contain private health information. 16.------Considerable mental concentration required.-- Continuous sitting or walking with smallamount of lifting or carrying.-- Moderateto heavy computer usage.17.------Average or periodic contact with conditions such asfumes, chemicals, hazards, diseases, and noises (possibly more than averagenoise due to intermittent operations by several employees completing theirduties in the same work location).18.------ Requires flexibility in work schedule.-- For daytime workers, this routinely entails a40-hour work week but including on-call hours and periodic extended hours dueto workload spikes.-- For evening andweekend social workers, this entails evening/nighttime shifts as well as on-callhours during the weekend.19.------ Occasional travel may be necessary.EDUCATION/TRAINING/EXPERIENCE:1.------------ LMSW required2.------------ Prefer a minimum of 3-5 years in hospital social work 3.------------ Moderate level of computer knowledge is required4.------------ Must have strong analytical and problem solving skills5.------------ Must have the ability to organize work and meetdeadlines6.------------ Strong interpersonal and communicating skills7.------------ BLS (CPR) to be obtained within 30 days of hire8.------------ Falls prevention and OSHA to be obtained within 30days of hire and renewable annually.--EQUIPMENT OPERATED:-------------------Computer-------------------Various--PHYSICALDEMANDS AND WORKING CONDITIONS:---------------------Medium Work asdefined by the U.S. Department of Labor constitutes a maximum lift of 21-50pounds on occasion and/or a maximum lift of 11-25 pounds on a frequent basis-------------------May be required to assist in the turning, lifting, ormoving patients as well as walking about from patients' rooms, as well as toand from various hospital areasSELECTION GUIDELINES:Formal application;verification of education, and experience; verification of license(s),certification(s), registration(s), accreditation(s) if applicable; oralinterview, reference and background checks; job related tests may be required.The duties listed above are intended only as illustrations of thevarious types of work that may be performed.--The omission of specific statements of duties does not exclude them fromthe position.
