The Social Worker is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of the patient s plan of care from admission to post-discharge. The Social Worker utilizes critical thinking skills, and the principles of case management to coordinate and implement a discharge plan that meets the patient s needs and ensures a seamless, effective and efficient transition of care across the continuum.

Job Requirements

EDUCATION/QUALIFICATION

*

* Current Licensure in Social Worker (LGSW/LCSW/LICSW) in the District of Columbia.

* Master's degree in Social Work

* Attendance and Punctuality Demonstrates the ability to arrive at work site on-time, limits the number of absences, provides adequate notice to supervisor for time off, takes lunch periods and breaks within prescribed times.

* Mandatory training, testing, and competencies completed on time Competency Training; Services Excellence Training; Annual Physical.

* Three years experience in acute care or other related healthcare field is preferred

* Case Management Certification preferred.

* Previous experience in case management and/or discharge planning is strongly preferred.

* Knowledge of government and third party reimbursement systems and contracts.

* Strong organizational and problem solving skills

* Ability to maintain confidentiality
