The Clinical Nurse Navigator is responsible for coordination and communication of care to Amputation Prevention Center® (APC) clinic patients throughout the course of treatment, from admission to discharge. You will monitor patients from inpatient to outpatient facilities, communicate with nursing and other staff members to ensure adherence to the clinical pathways, and coordinate with case management to track care plan and length of stay. As an essential part of this team, you will work to ensure the best outcome for our patients while developing educational opportunities for all those involved in patient care. What We Offer: Monday – Friday schedule, no weekendsComprehensive benefits package (Medical, Dental, Vision, Life, 401k)Generous paid time offInternal ongoing educational opportunitiesCompetitive compensation What You’ll Do: Implement and maintain APC clinical pathways in conjunction with all standard policies and proceduresPreform rounds on APC patientsTrack patients along the entire continuum of care, making recommendations and taking appropriate action to ensure APC successTrack program metrics, identify areas for improvement, and implement appropriate action plansMaintain ongoing communication with hospital staff from patient admission to dischargeWork alongside the Program Director to plan in-service educational lectures to partner skilled nursing, home health, and long-term medical staff Qualifications: Bachelors of Science in Nursing preferredCurrent, unencumbered license to practice as a Registered Nurse in the state employedMinimum 2 years’ healthcare experience in acute care hospital setting, admission coordination, and/or discharge planning preferredDemonstrated leadership, organization, and communication skillsBasic Microsoft Office skills (Word, Excel, Outlook) At RestorixHealth, we grow talent.
