Case Manager RN

Assess patient needs and develop individualized care plans in collaboration with multidisciplinary teams. Act as a patient advocate to coordinate required services or to resolve emergency problems in crisis situations. Track and evaluate patient recovery and modify care plans accordingly. Coordinate with healthcare providers, external agencies, and other stakeholders to streamline care transitions and secure necessary patient services. Educate patients and families about health status, care options, and community resources. Regularly review treatment plans with healthcare teams and adjust to achieve optimal patient outcomes. Strong interpersonal and communication skills Excellent organizational and time management skills Proven ability to make quick and accurate assessments of patient conditions Knowledge of healthcare laws, regulations, and standards Experience with electronic health record (EHR) systems Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN license valid in the state of employment Certification in Case Management (CCM) preferred
